How to Document & Submit CS Hours
Step One: Proposal/Basic Information
Students should provide the Community Service Coordinator with basic information regarding their project including what the project is, the organization, the supervisor and their contact information, and the location. Once this information receives approval, students can begin completing their hours.
If this information is not submitted to the Community Service Coordinator prior to completing hours, students are assuming the risk of hours NOT qualifying and NOT contributing to the graduation requirement.
Step Two: Completed Hours Log
Hours should be recorded each day a project is performed. Supervisors MUST sign off each time hours are completed. Logs without supervisor signatures will not be able to be processed.
Step Three: Reflection
Reflection is a crucial step of the community service process. Students should take the time to consider and express how the experience impacted themselves and the broader community. Additionally, reflection is a key evaluation tool for the community service program in terms of opportunities, effectiveness, and impact.
The reflection is a brief conversation between the student and community service coordinator (or other designated individual). There is also the option for this reflection to be written.
Timeline for Completion
The “Community Service - Project Documentation” form must be submitted to the Community Service Coordinator in a timely manner. If a project is expected to be ongoing and last for an extended period of time, hours should be submitted periodically (approximately every six months). Additional hour logs can be completed.

Forms
Please find the required documentation for community service hours below. The only form typically needed is the "Community Service Project Documentation." If a student chooses to write their reflection (as opposed to a conversation with the Community Service Coordinator), the "CS Written Reflection Form" should be completed. Additional hours may be recorded on the "CS Additional Hours Form."
The "In School Organizations" Form should ONLY be completed by advisors or LRUHS staff.
